Candidate Qualifying Information & Forms

Description
A brief description of the Candidate Qualifying process that includes a list of forms that must be filed with the Clerk when qualifying,.
City form - May be filed with Clerk any time prior to opening of qualifying period.
State Form DS-DE 9 - Must be filed with Clerk prior to opening a Campaign Bank Account.
State Form DS-DE 84 - Must be filed simultaneously with the DS-DE 9 form.
City form required by Code - Must be filed when qualifying.
State Form CE Form 1 - Generally referred to as Financial Disclosure Form. Must be filed when qualifying.
State Form DS-DE 24B - Must be filed when qualifying.
State Form DS-DE 105 - Must be filed when qualifying.
State Requirement - Notification of Public Logic & Accuracy Test of Election Equipment - Must be filed when qualifying.
City & State Requirement - Notice of Usage & Removal of Political Advertisements - Must be filed when qualifying
